REFER A YOUNG ADULT - Every Horizons Experience Begins with a Psychologist’s Referral

The Connie and Robert Foundation - Horizons program is the first of its kind in Québec to motivate youth to work through their psychological conditions.

Deadline for the application for the 2025 Horizons trip to France is December 20, 2024.

Who can refer a young adult to Horizons?

Licensed psychologists who are members of the Québec Order of Psychologists and who have obtained consent from their patient in advance can refer eligible candidates by completing an online application.

Please note: Out of respect for the privacy of the candidates, candidates must be referred by their psychologist. If you are not a candidate’s psychologist, please ask the candidate to visit our website or write us a message in the CONTACT section of this website. If you are a potential candidate, but are no longer in contact with your psychologist, please write us a message in the CONTACT section of this website.

Who is an eligible candidate?

Travel will be reserved for young adults, residents of Québec, between the ages of 21 to 25 years who have been followed by a psychologist for psychotherapy during adolescence (12-19 years) for a psychological condition and have made significant changes and have maintained gains as adults.

To be eligible, a candidate must not have received a prior trip or similar reward from any similar gift giving organization.

To be eligible, a candidate must have a valid passport (valid 6 months after June 2025).

Privacy: The Foundation and its Horizons program respect the privacy of our candidates, their families and treating psychologists. All medical and personal information is kept confidential and is not communicated with outside parties unless consent is provided by the candidate. Questions about referrals? Review Process

Following the deadline, an independent selection committee of five psychologists will review all applications received, contact referring psychologists and interview potential candidates. Questions about the candidate’s psychosocial demographics and their psychological journey will be asked during the interview.

Among all applications received by December 20, 2024, a group of five young adults will be selected by the end of January 2025 and will be informed of their selection shortly thereafter.

An initial group meeting will be held in April 2025. Selected participants, tour guide, chaperone volunteers and Founders of Horizons will be required to meet to prepare for the trip.

The trip is planned for June 15, 2025.

When I heard about the Horizons program for the first time, I was really touched by this beautiful initiative! It is so important to honor youth that make progress and surmount mental health difficulties! I quickly thought of a few young patients that I had the pleasure working with and whom I wished could benefit from this beautiful opportunity. Since I work with youth, I did not have contact with these patients for several years. I had to contact them to see where they were at on their personal journey and obtain their consent in order to refer them. I was very touched and happy to reach out to them and see that they were doing well after all those years. They were also touched that I thought about them for this incredible adventure.
